You only need to fill out a simple form that asks for name, address, birth date and a voter registration number. You will also be required to check boxes on the from to indicate that you are a U.S. citizen and will be 18 on or before election day. The form requests some optional information, such as your telephone number and email address. The form also asks you to list any previous address or name you have used on a former voter registration so that these records can be updated. You must sign the form.
